We are searching for a talented Advanced Practice Practitioner to join our Care Navigation Team. / Mobile Integrated Health program. 

 

Hours:  Full-time - 40 hours M-F 8A-4:30P

 

Shift:  Day – Standard – no weekend, holiday or on-call.

 

Location:  Fairhaven, MA

Requirements

  • Under Direction of the VP of Population Health and in collaboration with the CMO of SHN, provide an advanced level of clinical practice to deliver health care services to patients and families.
  • Provide advanced clinical assessments, diagnoses, and treatment plans for high-risk patients receiving care management per programing within SHN.
  • Provide clinical and operational oversight and assist in development of Southcoast's Mobile Integrated Health program.
  • Practice independently and in collaboration with appropriate physicians to provide care to patients and oversee conduct and or complete all required visits and documentation in accordance with CMS regulations for face to face and telehealth encounter visits.
  • Ensure effective communication and collaboration across the care teams.
  • Oversee and optimize care plans for high-risk patients with complex medical needs, ensuring continuity of care, timely interventions, and reduced hospital readmissions.
  • Lead implementation and ongoing management of clinical programs.
  • Monitor patient outcomes, conduct case reviews, and lead initiatives to improve care quality, patient satisfaction, and adherence to regulatory standards.

Qualifications

  • Nurse Practitioner: Master of Science Degree in Nursing is required. Certification from a National accrediting body to practice as a Nurse Practitioner is required.
  • Current Massachusetts R.N. registration is required.
  • Current Massachusetts and RI RN license registration is required.
  • Nurse Practitioner certificate is required.
  • Proven ability to care for adult patients with a broad range of physical and psychological problems is required.
  • Proven ability to demonstrate maturity, self-confidence, and the ability to follow complex patient management plans as directed by the attending physician is required.
  • Physician Assistant: Bachelors Degree in a related field is required.
  • Graduate of an A.M.A.-approved Physician Assistant program is required.
  • Certification from a National accrediting body to practice as a Physician Assistant is required.
  • The ideal candidate will possess substantial experience in emergency medicine, including direct collaboration with paramedics and other pre-hospital providers.
  • Familiarity with triage, stabilization, and the unique demand of emergent care is essential.
  • Qualified in all basic and advanced cardiac support standards is required.
  • Nurse Practitioner: Current license to practice in the Commonwealth of Massachusetts as a Registered Nurse. Certification (of current eligibility) issues by the Commonwealth of MA and the State of RI to practice as a registered nurse is required.
  • Current Nurse Practitioner license is required.
  • Must possess current CPR certification.
