We are searching for a talented Team Lead-Centralized Clinic Support

 

Hours: 40hrs

Shift: Day shift with every other weekend, holiday, and on-call rotation; 7:30am - 4:00pm

Location: Remote position - Must live in MA or RI

Requirements

Position reports to the Manager or designee. Perform a variety of duties involving the coordination of clinical support, personnel productivity monitoring and other activities as specified by department. Provide ongoing training, guidance, and support to nursing staff and other clinical staff to ensure compliance with clinical workflows, policy, procedure, and best practice standards. Monitor daily performance reports to measure staff performance and identify improvement opportunities. Collaborate with team manager to build and maintain daily staffing schedules to support daily operations. Perform diversified duties related to ambulatory nursing and clinical support programs to drive better health outcomes, including telephone triage, care coordination, medication refills, addressing preventative health care gaps, and other centralized services to support ambulatory patient care and population health management. Implement and monitor clinical workflows, escalate clinical performance concerns, and follow-up on quality and patient safety concerns. Collaborate with Professional Development staff to onboard, educate, assess competency of, and monitor clinical staff. In collaboration with practice leadership, ensure clinical practice of care to promote safe and high-quality patient care.

Qualifications

  • Minimum of five years nursing or relevant clinical experience required.
  • Three years of ambulatory care nursing/medical office experience preferred.
  • Bachelors degree in Nursing or related field highly desirable.
  • Supervisory experience preferred. 
  • Must possess and demonstrate excellent, effective interpersonal, communication and leadership skills and be able manage multiple priorities.
  •  Requires computer skills including but not limited to Microsoft Office and Electronic Health Record (EHR) experience.
  • Current RN licensure in the Commonwealth of MA and/or RI is required, based on assigned site.
  •  Dual licensure is required within 3 months of hire.
  • Certification in Ambulatory Nursing, Care Management or Care Coordination and Transitional Care desirable.
  • Current BLS certification is required.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired. 

  • Compensation: Pay rate will be determined based on level of experience.
