We are searching for a talented RN-LPN-Home Care Coordinator
Hours: 40hrs
Shift: Day shift, 8:00am - 4:30pm
Location: Visiting Nurse Association - Fairhaven, MA

Requirements
Under direction of the Transitional Care and Telehealth Manager or designee, perform diversified duties in support of the Post Acute Care Network (PACN) including but not limited to providing ongoing access to home health care for community residents , communicating with nursing facility regarding issues related to patient discharge process, scheduling in-service education programs for skilled nursing facility staff, providing consultation to the multi-disciplinary planning team, patients and/or their families and visiting clients in nursing facilities to contribute to plan of care. Work Cooperatively with all Southcoast Health staff including SPN, SPG, VNA and SHG, to oversee implementation of the PAC Program in community based facilities and to provide efficient transitions of care and improved quality outcomes.Qualifications
- Current licensure by Massachusetts & RI Board of Registration in Nursing (LPN or RN) is required.
- One to three years nursing experience is required.
- At least two years of medical-surgical experience in an acute care facility is required and home health experience preferred.
- Must possess and demonstrate highly effective interpersonal, customer service and communication skills.
- Requires proficient computer skills, including experience with various software, database and electronic records programs.
- Current CPR certification required.
- Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired
Compensation: Pay rate will be determined based on level of experience.
