We are searching for a talented RN-Case Coordinator
Hours: 32hrs
Shift: Day shift with weekend and holiday rotation; 7:30am - 4:00pm
Location: Case Management – Tobey Hospital – Wareham, MA

Requirements
Under the general supervision and direction of the Manager, Case Management or designee, function as a patient advocate. Manage resources. Conduct concurrent assessment and review. Apply organization standards, standing orders and/or guidelines specific to patients diagnoses. Communicate, consult and collaborate with patient/family/ representative, physician(s) payers and other members of the multidisciplinary healthcare team. Coordinate, facilitate and document transition planning activities. This position is part of Core Case Management staffing. It supports utilization management functions including physician to physician consultation for correct level of care determinations. This position also supports denial prevention and compliance with COP requirements.
Qualifications
- A Bachelors Degree in Nursing or other related healthcare field or the equivalent knowledge and experience is required.
- A comprehensive knowledge of Medical Terminology is required.
- Case Management certificate is preferred.
- One year of case management experience is preferred.
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Compensation: Pay rate will be determined based on level of experience.
