Position reports to the Practice Manager/Administrator or designee. Check-in and check-out patients at assigned practice(s). Perform various operational support functions including scheduling patient visits, tests and surgical procedures. Greet patients, answer and direct telephone calls. Perform various clerical and administrative functions. May cover different offices as needed or directed.

Under general supervision of individually assigned Manager, Patient Care Services or designee, perform a variety of duties including basic care, personal hygiene, mobility, nutritional support, maintain patient environment, and provide assistance to licensed staff in carrying out other related duties, etc.

Under the general supervision and direction of the Clinical Manager, Director, Supportive Care Services or designee, perform patient care activities in accordance with Southcoast VNA policies and procedures, communicate pertinent patient information and unusual findings to appropriate Clinical personnel, complete all required documentation accurately and in a timely manner and …
